Academic Internship

An internship is work experience undertaken for the purpose of learning in which the student assumes a responsible role in an organization and practices active reflection. Internships completed for academic credit have requirements and processes to follow. Under each section, you will find information and resources for the mentioned audience.

Site Supervisor - forms and assessment for internships for Academic Credit

Internship Learning Contract

  • Site supervisor will receive a jotform that has been filled out by the student intern and faculty sponsor. Site supervisor signature is required and final copies will be sent to all parties. The Internship Learning Contract is an agreement of the internship description, hours to be completed and learning outcomes.

Mid-Semester Evaluation

  • Site supervisor will receive a jotform that was initiated by the faculty sponsor. Site supervisor will evaluate the intern based on their performance thus far in the semester. Final copies will be shared with the faculty sponsor and site supervisor. It is recommended that the site supervisor also reviews the evaluation with the student to assist in their professional development.

Final Semester Evaluation

  • Site supervisor will receive a jotform that was initiated by the faculty sponsor. Site supervisor will evaluate the intern based on their performance throughout the semester. Final copies will be shared with the faculty sponsor and site supervisor. It is recommended that the site supervisor also reviews the evaluation with the student to assist in their professional development.
